Dr. Crouch has over 30 years of licensed experience as a Board-Certified Clinical Neuropsychologist. He has extensive experience with brain injury rehabilitation (acute, post-acute), psychiatric, general medical, and forensic populations. He has considerable research and training experience in a variety of academic settings including the University of California, San Francisco and the University of Connecticut Medical Center.

Dr. Crouch is a certified examiner for the Federal Aviation Administration (FAA), conducting both neuropsychological and psychological examinations. As a HIMS/AME neuropsychologist, he is qualified to conduct evaluations of pilots and air traffic controllers at all levels. These assessments are required for airmen with various conditions (e.g., SSRI usage, ADHD, HIV+,etc.) to obtain their medical certificates per federal guidelines.

It is a long established fact that a reader will be distracted by the readable content of a page when looking at its layout. The point of using Lorem Ipsum is that it has a more-or-less normal distribution of letters, as opposed to using ‘Content here, content here’, making it look like readable English. Many desktop publishing packages and web page editors now use Lorem Ipsum as their default model text, and a search for ‘lorem ipsum’ will uncover many web sites still in their infancy. Various versions have evolved over the years, sometimes by accident, sometimes on purpose (injected humour and the like).

FAA evaluations conducted by Dr. Crouch are not reimbursable by health insurance. This is because these assessments are, by definition, not “medically necessary” procedures and are strictly being conducted for FAA purposes. This is even the case if you are referred for the assessment by a physician. In fact, these evaluations are being conducted because the airman is reportedly functionally capable of engaging in aeronautical activities.

Full payment for Dr. Crouch’s services is due at the time of testing. He accepts a variety of payments including cash, check, or credit/debit card.

The cost of Dr. Crouch’s testing depends on the scope of evaluation required by the FAA. He will provide an estimate of cost after reviewing the referral letter from the FAA and getting additional details about your history. In general, the maximum cost of testing is $3500 with most assessments costing less.

It generally takes Dr. Crouch approximately 2 weeks from the time of testing until the evaluation report is completed and sent with supporting documentation to the FAA. However, he will do his best to work around your FAA deadlines. Please bring this up with him at the time of the initial interview.

The records and other paperwork needed for Dr. Crouch’s FAA evaluations depend on the type of evaluation that the FAA requires. To begin, please forward a copy of the letter you received from the FAA requesting that you undergo testing to Dr. Crouch at [email protected]. This letter will describe the types of records and other supporting documentation you will need to provide prior to testing. Dr. Crouch will review the letter and discuss this with you at the time of the interview.

Once you have gathered all relevant records, please forward them to Dr. Crouch via DropBox, OneDrive, or Google Drive link.

Like other FAA-approved neuropsychologists, Dr. Crouch is mandated by the FAA to conduct assessments in person. Consequently, it is not possible to conduct formal FAA cognitive or psychological testing virtually.
