Records
The FAA requires that you provide Dr. Crouch with records that are relevant to your assessment. This includes any documentation that may be pertinent to your reported history or condition, even if you’ve already provided it to the FAA. The FAA also requires that Dr. Crouch obtain particular types of records for airmen with a history of certain psychiatric diagnoses or treatment. A list of the required records may be in your FAA letter and/or can be provided by Dr. Crouch at the time of intake.
These records can include the following:
- Letters sent to you by the FAA
- Medical records
- Pharmacy records
- Mental health treatment/evaluation records
- Reports from prior psychological or neuropsychological testing
- Academic records (e.g., transcripts, IEP plans, 504 plans, etc.)
- Military Records
- Legal and arrest records
- Recommendation letters
- Personal statement
In general, records for your FAA evaluation should be sent to Dr. Crouch prior to your assessment. Use the form below to upload records so they can be forwarded directly to Dr. Crouch.
